New Patient Forms

Once you have been appointed for a consult with Dr. Walker, we will email our new patient forms to you.  You will be asked to complete these forms online and return them to us electronically prior to the date of your appointment.  Please look for an email from frontdesk@walkerperio.com.  The forms will be attached to that email with instructions, a map of our office, and contact information for the practice.

Insurance Plans Accepted

We accept select in-network dental insurance plans. If your plan is out of network, we're happy to file claims on your behalf, provided you have a PPO plan. Our team can help verify your benefits before your appointment.  Contact our office to verify your coverage and benefits before your visit.

Payment Policies

We accept most major credit cards, and we offer payment plans, including our in-house membership plan, as well as third-party financing through www.carecredit.com or Cherry.  Please ask us for details!
