New Patient Forms

Once you have been appointed for a consult with Dr. Walker, we will email our new patient forms to you.  You will be asked to complete these forms online and return them to us electronically prior to the date of your appointment.  Please look for an email from frontdesk@walkerperio.com.  The forms will be attached to that email with instructions, a map of our office, and contact information for the practice.

Insurances Accepted

Our practice is currently a fee-for-service practice.  However, we can and do submit dental claims on behalf of our patients under many dental plans.  We do not, however, accept plans that fall under a DMO/HMO umbrella. 

Payment Policies

We accept several major insurance plans and COBRA.  Please bring your insurance card to your initial visit.  We accept most major credit cards, and we offer payment plans, including an in-house membership plan as well as third-party financing through Care Credit and Lending Club.  Please call our office for more details.
